CellForge's current in-house Nurse Practitioner is licensed to practice in New York. Accordingly, clinical services provided by that practitioner are available only to eligible patients who are physically located in New York at the time healthcare services are rendered.

2. NO PROVIDER-PATIENT RELATIONSHIP FROM WEBSITE USE
Visiting CellForgeWellness.com, reading educational materials, purchasing a membership, purchasing a non-prescription product, completing a general questionnaire, or communicating with customer service does not, by itself, establish a healthcare provider-patient relationship.
A provider-patient relationship is established only through an appropriate clinical process with a healthcare professional legally authorized to provide care to you.

Where CellForge offers weight-management services, treatment decisions are made by licensed healthcare professionals following an appropriate clinical evaluation.
Not every patient is an appropriate candidate for medication-assisted weight management.
Availability of particular medications may depend upon clinical eligibility, applicable law, FDA requirements, pharmacy availability, drug shortages, and other regulatory considerations.

10. RESEARCH-USE-ONLY PRODUCTS
Products expressly identified by CellForge as “For Research Use Only,” “RUO,” or “Not for Human or Veterinary Use” are intended exclusively for legitimate laboratory, analytical, or scientific research.

12. RESEARCH PRODUCT CLAIMS
The presence of scientific studies concerning a compound does not establish that a CellForge research product is safe, effective, approved, or appropriate for human use.
